Voter registration forms are called "Applications for Addition to the Checklist" which you may get from your town clerk or download as a pdf file or a word document.
1. Complete the Application, on which you must include your Vermont Driver's License number. If you do not have a license or your license has been suspended, include the last four numbers of your social security number.
2. Take the Voters' Oath. It can be self-administered, requiring only that you attest that it has been taken, and sign the form if you have not voted in Vermont before.
3. Once completed, submit the form to the Town or City Clerk in the town or city in which you reside. If you mail the form or if are not yet 18 at the time of registration, you may complete the Voters' Oath at the polling place
on Election Day.
4. You can also register to vote when you register your car, get a driver's license or renew your registration or license.
5. Completed Applications must be received by your town clerk by 5 PM on the Wednesday preceding the day of election.
